Terms and conditions	By ticking this box I agree to the following: YOGABOWL only refunds in exceptional circumstances involving injury or severe illness. This pass is only valid for the current term, and do not carry into subsequent terms NOR extend to holiday classes. I will bring this pass to EVERY class with me. Lost passes incur a $25 replacement fee. At present, and for subsequent terms and classes I participate in, I hereby indemnify YOGABOWL and/or its instructors against responsibility for any accident, loss or injury suffered by me during the course of the activity. Should at any time, the supervisor consider that I require any medical treatment; I consent to your obtaining it at my expense. I am responsible for informing the instructor of any injuries or conditions that may effect my experience in class, before I begin.
